Default Added new "name decals" form
I added a new quickie form for the name decals ordering.

This form might go down occasionally to ease my work load (when I am busy with other things) - but I have been doing these exact decals all along, for those who have emailed me.

its a cool form, except the styling of the text, its all a bit times new roman
