My kid is learning with one of my old cars, the motor is too fast for him so I turned the throttle trim on the transmitter down to half which is perfect. But the motor is getting hot, do I need to put a smaller pinion on?

Is he crashing a lot and holding the throttle on? My lad does
Brushed or brushless? Slipper? |

motor draws more current at low rpm, id put in a slower motor or smaller pinion to let the motor rev more and keep the speed down

Yeah think thats it as its running at low rpm for long periods, will put a small pinion on it.

both my kids b4's are running 27 turn stock motors on lipo, geared 20/81. fast enough to entertain and slow enough not to break too often. no heat issues and around 20 mins on a 5ah lipo

Small pinion is the easiest fix. Also, turn the timing down a bit so the motor doesn't get too hot or become too 'cammy' - nothing, nothing, nothing then BAM - full power. HTH

Did you turn the epa down or did you actually turn the trim down??? Because that could also be the reason why you're generating heat in the motor, you are throttling up with the brakes slammed on then.
